All the sudden the required fields of your name and your email are appearing twice in my form.  I can go in and "hide" one of each, but can't delete any.  And even if i hide two they still come through in the emails to me.  This just started today.

This problem is still happening so I am wondering if my only hope is to rebuild a new form?  Of course this would mean losing all the saved forms online?

 I have the same problem. FD 40.1.63 - DNN 06.02.03.

That was something updated like 5 years ago. Sorry I would not be able to comment or remember what the cause was.

We have like 150 versions released since then.
